If you wish to order plaques for your loved ones, please fill out our Plaque Order Form at the church lobby and submit to the office together with your payment of $150 per plaque.

Check payments should be made to Hope Covenant Church, with the following info written in the memo: "HOPE Memorial Garden" and Honoree Name. You may also pay online by clicking this link.
